2026 Guide · Better patient care, new monthly revenue
Get paid every month for the care your patients need between visits.
Medicare reimburses your practice monthly for watching over chronic patients between appointments. This guide lists every 2026 code in plain English, and shows how a licensed clinical team we work with enrolls your patients with no upfront cost and none of your staff’s time.
Chronic Care Management code 99490 adds $66.13 a month, and Medicare allows both for the same patient.
Remote Patient Monitoring
Your patients send readings from home. Your practice sees them every day.
Patients use a connected blood pressure cuff, scale, glucose meter or pulse oximeter at home. Readings arrive automatically, a clinical team reviews them, and patients get a call when something changes.
You walk into the next visit with weeks of real numbers instead of one reading taken in the exam room, and your patients feel looked after between appointments.
Chronic Care Management
Monthly coordination for patients with two or more chronic conditions.
Chronic Care Management pays for the work between visits: keeping the care plan current, checking on medications, following up after a hospital stay, and calling patients each month.
It is built for your Medicare patients living with conditions such as diabetes, hypertension, heart disease or COPD. They get steady attention every month, and your practice is reimbursed for it.
How it works
Three steps, and your staff stays focused on the patients in front of them.
- You share your patient list. That is the only thing your practice provides.
- A licensed clinical team we work with enrolls your patients. They make the calls, explain the program, capture consent and set up devices.
- Care runs every month between visits.Your physicians stay in charge of every care plan.

Running both
Remote Patient Monitoring and Chronic Care Management work together.
Medicare allows both programs for the same patient in the same month, as long as the same minutes are not counted twice. Readings from home make the monthly care call more useful, and the monthly call keeps patients using their device.
Keith Kravitz has served physician practices with medical technology since 2002. On a 30-minute call he will walk through your patient panel and how enrollment would run at your practice.
